How to create a contact form in Patient Copilot?
Step 1: Navigate to the Form Builder 1. Log in to your Patient Copilot account.
- Navigate to the correct client’s account (aka sub-account)
- In the left-hand menu, click on Sites → Forms.
- Click the + Create New Form button to start a new form.!(https://assets.patientcopilot.ai/f0efc2ca27be7c68.png) You may choose to use one of our existing templates for “Contact Forms” or create a new one that suits your needs.!(https://assets.patientcopilot.ai/7b96be9af091d904.png)!(https://assets.patientcopilot.ai/045a1880f61928ab.png)!(https://assets.patientcopilot.ai/f868fe595d34ba58.png) ## Step 2: Add Form Fields (optional) Drag and drop the required fields from the Form Elements section, for example Full Name, Email, Phone Number, Message/Custom Text Area, etc. If you need additional fields, use: Custom Fields to collect unique data. Dropdowns, checkboxes, or radio buttons for specific options. You can also rearrange fields as needed by dragging them into the desired order. You can drag and drop required fields from the right sidebar into the form and rearrange them in the desired order. As you drag a field, Patient Copilot now provides smoother movement, a subtle shadow for clearer visual feedback, and automatic scrolling in longer forms so you can move fields higher or lower without scrolling manually. Toggle Full Width for responsive design.!(https://assets.patientcopilot.ai/75304ee362b13275.png) ## Form Actions: Under Settings, choose what happens after submission: Show a Thank You Message (Enter a custom message) or Redirect to Another Page (Enter a URL). Youi can also Trigger a Workflow or Automation (e.g., Send a follow-up email)!(https://assets.patientcopilot.ai/9a635f19fa67e2c5.png) ## Step 4: Save & Embed the Form Click Save Form once satisfied.
